Come along to our free workshop to find out how to complete the form and what information to include.

This will be a group session where you will …

• Be given information and advice to enable you to complete your DLA form
• Be given a practice copy of the form, on which you can make notes to help you complete your form with information which is specific to your child’s needs
• Receive a  step-by-step guide to take away with you, which will help you fill in your child’s form
• Have the opportunity  to ask our Parent Carer workers questions  about the form
• Have the opportunity to make a follow up appointment for a Parent Carer worker to check over your form before you send it
